How we see wound care billing.
Reviewed by the VisMed care settings & programs billing team
Each wound care encounter is a measurement problem. Debridement is coded by the deepest tissue removed and by the total area, and the note has to show both. Skin substitutes are billed by product code and units of area, with wastage rules and a coverage policy per product per payer that specifies wound type, duration, prior treatment and how many applications are covered. Hyperbaric oxygen has its own indications and supervision rules.
Skin substitutes are where the money and the scrutiny concentrate. Products are expensive, coverage policies are narrow and recently tightened, and payers audit applications against the policy. A practice that applies products outside the policy loses the product cost; one that documents to the policy is paid.
VisMed gives wound clinics measurement-driven templates, holds product-specific coverage rules per payer, tracks application counts per wound, and builds claims correctly for hospital-based and office settings.
Where wound care practices lose money
The patterns we find in the aging and denial history before anything else changes.
Debridement depth and area unsupported
Codes step by depth and by area. Notes without measurements or tissue description bill a level low or are denied.Skin substitutes outside policy
Wound type, duration, failed conservative care and application counts must meet the payer’s policy for the product. Outside it, the product is not paid.Units and wastage
Product billed by area units with wastage documented and the modifier where required. Errors are expensive per application.Setting errors
Hospital-based wound centers bill facility and professional components under different rules from an office. Wrong construction is denied.Same-day visit and procedure
The evaluation is billable with modifier 25 only when significant and separately documented. Routine wound assessments before debridement are not.

What a physician-owner in this specialty asks on the first call.
How does VisMed make sure skin substitute applications are paid?
By checking the payer’s policy for that product before application: wound type, duration, documented failed conservative care and the number of applications allowed. VisMed builds the note to the policy and tracks the count per wound.
What does the note need for debridement?
The deepest tissue removed, the area debrided, the instrument and the wound measurements before and after. VisMed’s templates capture them so the code billed is the code supported.
We are hospital-based. What changes?
The facility bills its component and the professional claim follows different rules on what is included. VisMed constructs claims by setting and reconciles both where we bill both.
How does VisMed handle product wastage?
Documented in the note with the amount used and discarded, billed with the wastage modifier where the payer requires it, and reconciled to product inventory.
Does VisMed bill hyperbaric oxygen therapy?
Yes. Hyperbaric oxygen is covered for a short list of indications with documentation and supervision requirements that payers check closely. VisMed confirms the indication against the plan’s policy before the first treatment and bills the physician supervision and, where applicable, the facility component per session.
