Chronic care management

Recurring revenue, documented to survive an audit. A compliant CCM program run properly: eligible patients identified and enrolled, care-coordination time tracked, and documentation built to withstand scrutiny rather than to look complete.

01How we run it

How VisMed runs chronic care management.

Reviewed by the VisMed programs & staffing team

We run chronic care management as a program with our own staff, because that is the only version of it that survives contact with a busy clinic. Most practices that try CCM stop within a year: the consent, the care plans, the time logs and the monthly outreach land on nurses who already have a full day, and the revenue is not worth the friction.

So our care managers do the work, in your system, under your physicians’ general supervision. Eligible patients are identified from your panel, consented and enrolled; care plans are built and kept current; monthly time is logged against the threshold as it happens, not reconstructed; and the claims go out with documentation that reads as a record of care rather than a justification for a bill.

For internal medicine, geriatrics, endocrinology, nephrology and cardiology practices, CCM is usually the largest recurring-revenue line the practice is not currently billing.

03Questions

Chronic care management, asked and answered.

What a practice asks about this service on the first call.

Who does the care management work at VisMed?

VisMed’s clinical care managers do it, under your physicians’ general supervision as Medicare requires. Consent, care plans, monthly outreach, time logging and the claims are handled by VisMed staff working inside your system.

Is VisMed’s CCM documentation audit-ready?

VisMed logs time as it is spent, keeps care plans current and consent on file, and structures every month’s record so it reads as care delivered rather than a justification assembled at billing time.

Which patients qualify for CCM through VisMed?

Medicare patients with two or more chronic conditions expected to last at least a year, who consent to the program. VisMed identifies them from your panel and reports how many are enrolled, active and billed each month.
