Insurance Eligibility Verification Services for US Healthcare Providers

Insurance eligibility verification is confirming, before the visit, that a patient’s plan is active, that it covers what you are about to do, and what the patient will owe. VisMed verifies eligibility and benefits for every appointment, inside your own EHR, so coverage problems are solved at the front desk instead of coming back as denials.

01Overview

What our insurance eligibility verification services include

Reviewed by the VisMed revenue cycle team

An eligibility denial is the most avoidable denial there is, and one of the most expensive. The claim fails after the care has been delivered and the patient has gone home, often weeks later, when the patient is harder to reach and the filing window is already running. Everything that could have prevented it was knowable before the visit.

Our insurance eligibility verification services check each patient’s coverage while there is still time to do something about it. Every scheduled appointment is verified two business days ahead. Walk-ins and same-day additions are verified in real time at registration. Medicaid and marketplace plans, where coverage can change from one month to the next, are checked again on the day of the visit.

We verify more than active coverage. Medical insurance eligibility verification only tells you the plan exists; benefits verification tells you what it pays for. We check both, record the answers in your practice management system, and give the front desk a patient responsibility estimate before the patient walks in. We provide healthcare insurance eligibility verification services for physicians, group practices, hospitals, therapists, clinics and laboratories across the USA.

02Both checks

Insurance eligibility vs. benefits verification: why we check both

Eligibility verification answers one question: is this patient covered by this plan on the date of service? Benefits verification answers the next ones: what does the plan pay for this service, what does the patient owe, and what has to happen first for the claim to be paid? A patient can be fully eligible and the visit can still be denied, because the service was excluded, the visit limit was used up, or an authorization was never requested.

That is why we check both on every patient. An active plan is the start of the answer, not the whole answer. Insurance eligibility and benefits verification together is what lets the front desk collect the right amount, and what keeps the claim from coming back.

Eligibility verification

  • Is the plan active on the date of service?
  • Do the coverage dates and plan type match what we have?
  • Is the member ID, group number and subscriber correct?
  • Is the patient a dependent on someone else’s plan?
  • Which payer is primary, and which is secondary?

Benefits verification

  • What are the copay, deductible and coinsurance?
  • How much of the deductible is left this plan year?
  • Is this service covered, limited or excluded?
  • How many visits are allowed, and how many are used?
  • Does it need prior authorization or a referral?
03Checklist

What we verify for every patient

Ten answers recorded for every appointment, each from the source that gives it reliably.

Active coverage

What we recordActive, inactive or terminated, with the payer’s response on file

Where it comes fromElectronic eligibility check, confirmed by portal or call

The first question and the most common reason for an eligibility denial. A plan that ended last month still prints a valid-looking card.

Coverage dates

What we recordPolicy effective date and termination date

Where it comes fromEligibility response

Coverage has to be active on the date of service, not the date you checked. New jobs, open enrollment and Medicaid redeterminations all move these dates.

Plan type

What we recordHMO, PPO, EPO, POS, Medicare Advantage, Medicaid managed care

Where it comes fromEligibility response and insurance card

The plan type decides whether a referral is needed, whether out-of-network care is paid at all, and which rules the claim will be judged by.

Network status

What we recordIn network or out of network, for your NPI and location

Where it comes fromPayer portal or call

A plan can be active and still pay nothing, or much less, if you are out of its network. We check against your billing and rendering NPI, not only the group.

Copay

What we recordCopay for this visit type: office, specialist, therapy, urgent care

Where it comes fromEligibility response

The amount the front desk should collect at check-in. Specialist and therapy copays often differ from the primary care copay printed on the card.

Deductible

What we recordIndividual and family deductible, met to date and remaining

Where it comes fromEligibility response, call when blank

Early in the plan year most of the visit may fall to the patient. The remaining deductible is what turns a copay into a realistic estimate.

Coinsurance

What we recordPatient percentage after the deductible, and the out-of-pocket maximum

Where it comes fromEligibility response

Coinsurance is where patient balances grow. Knowing the percentage and how close the patient is to their maximum makes the estimate accurate.

Visit limits

What we recordVisits allowed per plan year, visits used, benefit limits

Where it comes fromPayer portal or call

Therapy, chiropractic and behavioral health plans often cap visits. Once the limit is reached, the next visit is the patient’s cost unless an extension is approved.

Authorization needs

What we recordPrior authorization or referral required, by service or CPT code

Where it comes fromPayer portal, policy lookup or call

A missing authorization is a denial no appeal can fix. We note what is required and when it expires, so it is in place before the visit.

Primary and secondary payers

What we recordPayer order, secondary and tertiary plans, coordination of benefits status

Where it comes fromEligibility responses from each payer, patient questionnaire

The wrong payer order denies the claim with both payers. We settle the order, and flag any coordination of benefits update the patient still owes their insurer.

04Process

Our insurance eligibility verification process

Eight checkpoints from the insurance card to the daily report, run in five steps. Each one has an owner on our side.

  1. 01Patient information
  2. 02Insurance card
  3. 03Eligibility check
  4. 04Benefits check
  5. 05Payer call
  6. 06Patient responsibility
  7. 07System update
  8. 08Report

Step 1: Patient information collection

Accurate verification starts with accurate data. We review the patient intake and the insurance card, front and back, and check the member ID, group number, subscriber name and date of birth, relationship to the subscriber, and any second policy the patient carries. Most eligibility rejections start with a typo here.

What we doReview intake and insurance card details, correct demographics, ask about other coverage

What you doCollect the card and photo ID at scheduling or check-in

Step 2: Real-time eligibility and benefits checks

Each patient is run through an electronic eligibility check, the standard EDI 270/271 transaction, through your clearinghouse or practice management system. The response confirms active coverage and returns benefits for the service type. We read the response rather than trusting a green checkmark, because a plan can return active while the details that matter are missing.

What we doRun electronic eligibility checks two business days ahead and on the day for walk-ins

What you doNothing; checks run against your schedule

Step 3: Payer calls and portal verification

When the electronic response is incomplete, conflicting or silent on the service you are providing, we go to the payer portal or call the payer. Live-call verification gets the answers an automated check cannot: visit limits, carve-outs for behavioral health or therapy, authorization rules by code, and out-of-state Blue plan benefits. We note the representative’s name and the call reference number.

What we doVerify through payer portals and live calls, document every answer and reference number

What you doNothing

Step 4: Patient responsibility calculation

With coverage and benefits confirmed, we estimate what the patient owes for the visit: copay, the share of the deductible still to be met, and coinsurance, against your fee schedule or the payer’s allowed amount. The estimate goes where the front desk will see it at check-in.

What we doCalculate the patient responsibility estimate for each visit

What you doCollect at the point of service, or discuss a payment plan under your policy

Step 5: EHR updates and verification reports

Results are entered into your EHR or practice management system: coverage, benefits, payer order, authorization needs and the estimate, attached to the appointment. Every day you get a verification report listing what was verified, what needs attention, and which patients need to bring a new card, update their coordination of benefits, or be told the visit may be self-pay.

What we doUpdate the patient’s record and appointment, send the daily verification report

What you doAct on the flagged patients before their visit

05Two methods

Real-time and live-call insurance verification

Real-time insurance eligibility verification is fast, cheap and right most of the time. An electronic check returns active coverage and the main benefits in seconds, and for a routine office visit with a large commercial payer or Medicare, that is usually all you need.

It is not always enough. Some payers return a bare “active” with no benefit detail. Behavioral health and therapy benefits are often carved out to another company that the electronic response does not show. Out-of-state Blue plans, workers’ compensation, and many Medicaid managed care plans need a portal lookup or a phone call. Our verification specialists use real-time checks first and live calls when the answer is not complete, so speed never comes at the cost of a denial.

06Who we serve

Who we provide eligibility verification services for

From one physician’s schedule to a hospital outpatient department. The questions are the same; the volume, payers and plan rules are not.

  1. Physicians and individual providers

Eligibility verification for physicians, nurse practitioners and physician assistants without a dedicated front office. We verify the schedule before you see it, so the only coverage surprises are the ones we have already told you about.
  2. Small and medium practices

Eligibility verification for small practices and growing groups whose front desk is already answering phones, checking in patients and collecting copays. We take verification off their list and put the results in your system.
  3. Large medical groups and hospitals

Eligibility verification for medical groups and hospital eligibility verification for outpatient departments and employed physicians: high daily volume, many locations and payers, and verification reports by location and provider.
  4. Mental and behavioral health therapists

Behavioral health insurance verification and mental health insurance verification for psychologists, counselors, social workers and psychiatric practices: carved-out behavioral benefits, session limits, telehealth coverage and authorizations for ongoing care.
  5. Physical therapists

Physical therapy insurance verification for PT, OT and speech therapy: visit limits per plan year, combined therapy caps, authorization after the evaluation, and re-verification across a plan of care.
  6. Clinics and medical laboratories

Urgent care insurance verification in real time for walk-ins, and laboratory insurance verification for clinical labs, where the patient is often never seen in person and coverage, payer order and medical necessity rules have to be right from the requisition.
07Payers

Insurance payers we verify

We verify Medicare and Medicaid eligibility, commercial insurance, and the plans in between: Medicare Advantage, Medicaid managed care, marketplace plans, military and veterans programs, workers’ compensation and auto claims. Government payers and private payers each have their own portals, rules and phone trees; our insurance verification specialists work them every day.

Payer names are trademarks of their owners. VisMed is not affiliated with or endorsed by any payer.

08Problems we solve

Eligibility problems we solve

Where coverage problems usually turn into lost revenue or unhappy patients, and what we do about each one.

  1. Eligibility-related claim denials

Coverage terminated, patient not found, wrong member ID, service not covered: eligibility denials sit near the top of most practices’ denial lists, and almost all of them were preventable. Verifying every patient before every appointment, and reading the answer rather than trusting a checkmark, is how we get you fewer eligibility-related denials.

  2. Inactive or terminated coverage

Patients change jobs, lose Medicaid at redetermination, or miss a marketplace premium and fall into a grace period. When we find inactive or terminated coverage, the front desk hears about it before the visit, with time to ask the patient for new insurance, find other coverage, or agree a self-pay rate.

  3. Missed prior authorization requirements

A service performed without a required authorization is usually denied for good, and the patient often cannot be billed for it either. We check authorization and referral requirements for the planned service as part of every verification, and hand them to the authorization desk while there is still time.

  4. Wrong primary and secondary payer order

A Medicare patient who is still working, a child on both parents’ plans, a car accident: coordination of benefits mistakes deny the claim with both payers and take weeks to untangle. We identify every policy, apply the payer order rules, and tell the patient when their insurer needs a coordination of benefits update.

  5. Surprise balances for patients

Nothing damages patient satisfaction like a bill that arrives a month later for an amount nobody mentioned. An estimate before the visit gives patients upfront cost transparency, gives the front desk a number to collect, and leads to better point-of-service collections and fewer balances sent to statements.

09Why VisMed

Why outsource insurance eligibility verification to VisMed

In-house vs outsourced eligibility verification is mostly a question of time. Verification done properly takes minutes per patient, longer when a payer has to be called, and it usually lands on a front desk that is already checking patients in. When the desk gets busy, verification is what slips. Outsourcing insurance eligibility verification to a partner gives you dedicated specialists who verify every patient before every visit, and reduced front-desk workload for your own team.

  1. Verification before every appointment

Every scheduled patient is verified two business days ahead, walk-ins and same-day additions in real time, and plans that change month to month are checked again on the day. Not a sample, and not only new patients.
  2. Trained insurance verification specialists

Our insurance verification specialists know the portals, the phone trees and the plan rules, and they know when an electronic “active” is not the whole answer. The same team works your payers every day, so they learn your mix.
  3. Works inside your EHR and PM system

We verify and record results in the EHR, EMR or practice management system you already use, including athenahealth, eClinicalWorks, Epic, NextGen, AdvancedMD, Tebra, ModMed, WebPT and SimplePractice. Your front desk sees the answers where they already look.
  4. HIPAA-compliant verification

We sign a Business Associate Agreement (BAA) before we start, work inside your systems, and keep no PHI on our own servers. Access is limited to the people verifying your patients.
  5. Daily verification reports

A short report every day: patients verified, coverage problems found, authorizations needed, and patients who need a call before their visit. Monthly, we show eligibility denials trended over time so you can see the result.

11Questions

Insurance eligibility verification FAQs

What providers ask about eligibility verification, and about working with VisMed, on the first call.

What do your insurance eligibility verification services include?

VisMed’s insurance eligibility verification services include patient eligibility checks, benefits verification, primary and secondary coverage verification, prior authorization and referral requirement checks, patient responsibility estimates, re-verification for recurring visits, updates to your EHR or practice management system, and a daily verification report.

How much do outsourced eligibility verification services cost?

When VisMed runs your revenue cycle, eligibility verification is included in the percentage of collections, with no separate charge. As a standalone service, insurance verification pricing depends on your monthly verification volume and payer mix, and VisMed quotes it in writing before work starts.

How quickly can you verify a patient’s insurance?

VisMed verifies scheduled appointments two business days ahead. Walk-ins and same-day additions are verified in real time at registration, with an electronic answer in seconds. When a payer has to be called, the answer usually comes back within the same working session, depending on the payer’s hold times.

Do you verify both eligibility and benefits?

Yes. VisMed confirms active coverage, coverage dates, plan type and network status, and verifies benefits: copay, deductible, coinsurance, visit limits, exclusions and authorization requirements for the planned service.

Do you handle same-day or urgent verifications?

Yes. Same-day appointments, walk-ins and urgent (stat) verifications are handled as they come in during your clinic hours, with an electronic check first and a payer call when the electronic answer is not complete.

Can your team work inside our EHR or practice management system?

Yes. VisMed works inside the system you already use, including athenahealth, eClinicalWorks, Epic, NextGen, AdvancedMD, Tebra, ModMed, WebPT and SimplePractice, and records verification results on the patient and the appointment.

Do you verify Medicare, Medicaid, and commercial insurance?

Yes. VisMed verifies Medicare and Medicare Advantage, Medicaid and Medicaid managed care plans in every state, commercial payers including UnitedHealthcare, Blue Cross Blue Shield, Aetna, Cigna and Humana, marketplace plans, TRICARE, VA Community Care and workers’ compensation.

What happens if a patient’s insurance is inactive or can’t be verified?

VisMed flags the patient on the daily report before the visit, with what was found. Your front desk can then ask the patient for new insurance details, check for other coverage such as Medicaid, or set the visit up as self-pay under your policy, before care is delivered.

Is outsourced insurance verification HIPAA-compliant?

Yes. VisMed signs a Business Associate Agreement before starting, works inside your own systems, keeps no PHI on its own servers, and limits access to the specialists verifying your patients.

Why outsource eligibility verification instead of handling it in-house?

Outsourcing eligibility verification gives you specialists who verify every patient before every visit, including the payer calls a busy front desk skips. That means fewer eligibility denials, better point-of-service collections and less front-desk workload, while the results stay in your own system.
