FQHC billing

A health center is paid per qualifying encounter at a rate someone negotiated years ago, and it has to prove every encounter qualifies. The billing is the reconciliation, and we run it per program and per state.

01Our view

How we see fqhc billing.

Reviewed by the VisMed care settings & programs billing team

Federally qualified health centers are paid a prospective rate per qualifying visit under Medicare and, in most states, an encounter rate under Medicaid, with wrap-around payments that reconcile managed-care payments up to that rate. What counts as a qualifying visit, how many can be billed per day, which services are carved out and billed fee-for-service, and how the wrap is claimed differ by program and by state.

Underneath the encounter rates the center still has to code every service correctly, because commercial payers pay fee-for-service, because Medicare requires the underlying codes on the claim, and because quality reporting depends on them. And the center has to apply a sliding fee schedule to uninsured patients and report on it.

VisMed applies encounter qualification rules per program and state at charge entry, codes every encounter fee-for-service underneath, reconciles the wrap visit by visit, and keeps the data the center owes its funders clean.

02Where it leaks

Where fqhc practices lose money

The patterns we find in the aging and denial history before anything else changes.

  1. Encounters that do not qualify

Nurse-only visits, some same-day combinations and certain services do not qualify for the encounter rate. Billed as encounters, they are denied; billed wrong the other way, revenue is lost.
  2. Wrap-around not reconciled

Managed-care payments below the encounter rate are made up through wrap claims or reconciliations that many centers file late or incompletely.
  3. Multiple encounters on one day

Medical, dental and behavioral health encounters on the same day may each be billable under the state’s rules. Centers that bill one leave the others.
  4. Carve-outs billed under the rate

Services carved out of the encounter rate are paid fee-for-service and have to be billed that way, with the codes and modifiers the program expects.
  5. Underlying coding weak

Because the encounter pays a flat rate, coding discipline slips, which costs money on commercial claims and damages quality reporting.
03How we run it

What we do differently for fqhc

Specialty-matched coders and a denials specialist, working inside your own system.

  1. We apply qualification rules per program

Medicare, each state Medicaid and managed-care plan qualification rules applied at charge entry, with multiple-encounter days handled per state.
  2. We reconcile the wrap

Managed-care payments compared with the encounter rate per visit and wrap claims or reconciliations filed on schedule.
  3. We code fee-for-service throughout

Every encounter coded completely, so commercial claims, Medicare claim lines and quality reporting are right.
  4. We support sliding fee and reporting

Sliding fee discounts applied and documented in your system, with the data the center needs for its reporting.

05Questions

FQHC billing, asked and answered.

What a physician-owner in this specialty asks on the first call.

How does VisMed determine whether a visit qualifies for the encounter rate?

From the program’s rules: the type of provider, the services rendered, and the same-day combinations each program and state allows. VisMed holds the rules per payer and applies them at charge entry, so encounters are billed as encounters and carve-outs as fee-for-service.

Can VisMed handle our Medicaid wrap-around?

VisMed compares managed-care payments with the encounter rate visit by visit, and files the wrap claims or periodic reconciliations your state uses on schedule with supporting detail.

Does VisMed bill dental and behavioral health too?

VisMed bills dental on the dental code set and behavioral health under its rules, with the same-day encounter logic your state applies.

Does a flat encounter rate mean coding matters less?

No. Commercial payers pay fee-for-service, Medicare requires the underlying codes, and your quality reporting depends on them. VisMed codes every encounter completely.

Does VisMed support our UDS reporting?

VisMed keeps the encounter, diagnosis and service data that the Uniform Data System draws on clean at the source, and gives your reporting lead extracts by table so the annual submission is a reconciliation rather than a reconstruction.
