Infectious disease billing

Infectious disease is consultative medicine with almost no procedures. The claim is the note, and the note has to carry the level. We help it do that.

01Our view

How we see infectious disease billing.

Reviewed by the VisMed medical specialties billing team

An infectious disease physician spends the day thinking, reading records and talking, in hospitals and clinics, and bills almost entirely evaluation and management. There is no procedure revenue to make up for an underleveled visit. If the decision-making or the time is not in the note, the money is not in the claim.

The hospital side has its own rules: initial and subsequent visits, consultations where payers still recognize them, critical care time, and the place-of-service and same-day rules that cause cross-setting denials. Outpatient antimicrobial therapy adds infusion, drug and supervision billing. HIV and hepatitis care adds long-term medication management with authorization and care-management opportunities.

VisMed builds infectious disease accounts around documentation: templates that capture time and decision-making, coders who read the note, and a daily hospital census reconciliation that makes sure every visit becomes a claim.

02Where it leaks

Where infectious disease practices lose money

The patterns we find in the aging and denial history before anything else changes.

  1. Complex visits leveled low

Multi-drug regimens, resistance data and outside-record review are high-complexity decision-making. Notes that read as results lists level down.
  2. Time not documented

Long consultations and family discussions support time-based levels and prolonged services only if total time is recorded.
  3. Hospital visits unbilled

Rounding lists that do not reconcile to claims lose visits, particularly for patients seen across multiple facilities.
  4. Outpatient antimicrobial therapy

Infusion, drug, line care and supervision have separate codes and payer rules. Practices often bill only the visit.
  5. Telehealth place-of-service and modifiers

ID does a lot of telehealth, and each payer has its own place-of-service and modifier conventions. Wrong ones are denied.
03How we run it

What we do differently for infectious disease

Specialty-matched coders and a denials specialist, working inside your own system.

  1. We give you documentation templates

Total time and the elements of decision-making captured with minimal typing, in hospital and clinic templates.
  2. Our coders read the note

Every encounter is leveled from the documentation, with a query when the note is close to a higher level.
  3. We reconcile rounding daily

A daily census across every facility you round at, matched to claims, so no hospital visit goes unbilled.
  4. We hold telehealth rules per payer

Place-of-service and modifier conventions held per plan and applied at charge entry.

05Questions

Infectious disease billing, asked and answered.

What a physician-owner in this specialty asks on the first call.

How does VisMed make sure hospital visits get billed?

With a daily census reconciliation across every facility you round at. Each patient-day on the list becomes a claim or a documented reason, and VisMed works the gaps the same week.

Should our physicians document time?

Yes, total time on the date of service, every visit. In infectious disease it frequently supports a higher level than decision-making alone and unlocks prolonged services. VisMed’s templates make it a single line.

Can VisMed bill outpatient antimicrobial therapy?

VisMed bills infusion administration, the drug where the practice supplies it, line care and physician supervision under the applicable codes and payer rules.

Does VisMed handle e-consults and telehealth?

Yes, with each payer’s place-of-service and modifier conventions applied at charge entry, and interprofessional consultation codes where the payer recognizes them.

How does VisMed handle payers that no longer recognize consultation codes?

We hold the rule per payer. Where a plan has dropped consultation codes, VisMed bills the visit under the initial or subsequent hospital or office code the plan accepts, at the level the note supports, so the claim is neither downcoded nor denied for an obsolete code.
