Pediatrics billing

Pediatric billing is a vaccine problem and a Medicaid problem wearing a well-child schedule. We get those two right first; the rest of the cycle is ordinary.

01Our view

How we see pediatrics billing.

Reviewed by the VisMed primary & acute care billing team

A pediatric practice’s revenue sits in the well-child schedule and in immunizations, and both come with rules that no other specialty faces. Vaccine administration is coded per component, with counseling, and Vaccines for Children stock is billed differently from private stock. A practice that gets administration wrong on a busy well-child day loses the cost of the visit without ever seeing a denial.

On the payer side, a large share of most panels is Medicaid, CHIP and their managed-care plans, and each state runs them differently. Eligibility changes month to month, newborns appear under a parent’s coverage and then move, and filing limits are shorter than commercial norms.

VisMed staffs pediatrics with people who have billed it. They know the schedule, the vaccine tables and your state’s Medicaid plans, and they check eligibility before the child is in the room rather than after the claim comes back.

02Where it leaks

Where pediatrics practices lose money

The patterns we find in the aging and denial history before anything else changes.

  1. Vaccine administration under-counted

Administration for patients under 19 with counseling is billed per component, not per injection. A combination vaccine can carry several components. Practices that bill one administration line per shot leave the rest unbilled on every well visit.
  2. VFC stock billed as private stock

Vaccines for Children product is supplied by the state and cannot be billed to Medicaid as product; only administration is paid. The reverse mistake, giving private stock to a VFC-eligible child, costs the practice the vaccine.
  3. Newborn coverage not established

Newborns are covered under the mother’s plan for a limited period, then need their own enrollment. Claims for the first visits go out to the wrong member and come back as eligibility denials weeks later.
  4. Sick visit on a well-child day given away

A well-child visit where an ear infection is also diagnosed and treated is two services. Without a separately documented problem visit and modifier 25, the second is lost.
  5. Developmental and behavioral screens unbilled

Standardized screening instruments have their own codes and are expected at defined ages. Practices that administer the screens but do not bill them leave a line off every visit.
03How we run it

What we do differently for pediatrics

Specialty-matched coders and a denials specialist, working inside your own system.

  1. We maintain the vaccine tables for you

Product codes, component counts, administration codes and VFC eligibility live in a table your VisMed account team keeps current, so the claim is built right at charge entry rather than corrected later.
  2. We hold state Medicaid and CHIP rules per plan

Each managed-care plan on your panel has its own eligibility, referral and filing rules in our pre-submission checks. We enroll the practice and each provider with every plan you see.
  3. We verify eligibility before the visit, newborns included

Two-day-ahead verification with a newborn workflow that confirms whose plan the baby is on before the first claim goes out.
  4. We reconcile the schedule to claims daily

Every well-child slot on your schedule ends as a claim with the screening, vaccine and problem-visit lines it should carry. We close the list every day.

05Questions

Pediatrics billing, asked and answered.

What a physician-owner in this specialty asks on the first call.

Does VisMed know our state’s Medicaid and CHIP plans?

VisMed bills pediatrics across many states, and the account team assigned to you is set up on your state’s plans before your first claim goes out. If a plan is new to us, we learn its rules during onboarding, not on your denials.

How does VisMed code vaccine administration?

Per component, with counseling codes for patients under 19, and with VFC stock billed as administration only. VisMed maintains the table that drives this and reconciles your vaccine inventory against billed administrations so no shot goes unbilled.

How does VisMed handle parents who owe balances?

Pediatric balances are small and numerous, so VisMed posts accurately, sends clear statements, and sets up card-on-file and payment plans through your system, so your front desk is not chasing small amounts at check-in.

Can VisMed bill a sick visit and a well visit on the same day?

When the sick problem is significant and documented separately, VisMed bills both with modifier 25 on the problem visit, and we check the note supports it. Some Medicaid plans have their own rules on this, and we apply them per plan.

What does VisMed do with newborn claims before the baby has coverage?

We hold them briefly, confirm the enrollment with the parent’s plan or the state, and submit to the right member the first time, inside the filing window. That avoids the eligibility denial and the rework.
