How we see hematology billing.
Reviewed by the VisMed medical specialties billing team
Benign and malignant hematology share the same billing mechanics: high-cost drugs given in the office, authorized per course, billed by units with wastage and NDC rules, with administration codes built from the infusion record. Iron infusions, factor products, immunoglobulin and anticoagulation management add their own coverage criteria and frequency rules.
The visits are data-heavy, with lab review and management decisions that support higher E/M levels than most practices bill, and the ongoing management of chronic conditions supports care-management programs.
VisMed brings the same drug desk and coding discipline we use for oncology to hematology, plus a leveling review that makes sure the complexity of the visit is reflected in the claim.
Where hematology practices lose money
The patterns we find in the aging and denial history before anything else changes.
Iron and immunoglobulin outside criteria
Coverage depends on lab values, prior therapy and frequency. Doses given outside the plan’s criteria are not paid, and the practice bought the drug.Units, wastage and NDC errors
Per-unit drug billing with vial-size mismatches, wastage rules and NDC formats fails at the line level on expensive drugs.Administration hierarchy
Initial, sequential and concurrent services, and push-versus-infusion by time, have to be built from the record.Complex visits leveled low
Interpreting a bone-marrow result and adjusting therapy is high-complexity decision-making. Notes that report results without reasoning level down.Procedures and lab handling
Bone-marrow biopsies, therapeutic phlebotomy and in-office lab draws have their own codes and are commonly under-captured.
What we do differently for hematology
Specialty-matched coders and a denials specialist, working inside your own system.
Our drug desk runs each course as a case
Authorization with the plan’s criteria checked, expiry tracked, and reimbursement compared with acquisition before the chair is booked. We build claims from the infusion record
Administration and drug lines come from start and stop times and the drug sequence, with units reconciled to inventory. We review leveling
Our coders read complex visits against the decision-making table and give the physician short feedback. We capture procedures
Biopsies, phlebotomy and injections are prompted at charge entry and checked against the notes.

What a physician-owner in this specialty asks on the first call.
How does VisMed handle iron infusion coverage?
VisMed checks each plan’s criteria, including lab thresholds and prior oral therapy, before the infusion is scheduled, puts the authorization in place with an expiry we track, and compares the allowed rate with your cost per product.
Does VisMed bill administration the same way as oncology?
Yes. The hierarchy of initial, sequential and concurrent services, hydration and push rules are built from the nursing record by VisMed’s certified coders, with a second review on drug lines.
Can VisMed bill bone-marrow procedures done in the office?
Yes, with the correct codes for aspiration and biopsy, imaging guidance where used, and the pathology hand-off tracked so the specimen is billed by the right party.
Is care management applicable to benign hematology?
Often. Chronic anemia, clotting disorders and anticoagulation management involve ongoing coordination that fits the program requirements. VisMed sets up consent and time tracking so it is billable.
How does VisMed bill factor products for bleeding disorders?
Factor is billed by units with the NDC and, on most plans, a standing authorization tied to the patient’s diagnosis and dosing plan. VisMed keeps that authorization current, reconciles dispensed units to the assay values on the vials, and checks the plan’s allowed rate against your acquisition before each order.
