How we see home health billing.
Reviewed by the VisMed care settings & programs billing team
Under the patient-driven grouping model, each thirty-day period is paid at a rate determined by the primary diagnosis, the referral source and timing, the functional score from the assessment and comorbidities. Small differences in coding and assessment move periods between payment groups. Visits below a threshold turn the period into a per-visit payment. The agency has to get the assessment right and code it right to be paid what the care is worth.
Before any of that pays, the physician must document a face-to-face encounter, sign the plan of care and certify eligibility, and the agency must file a notice of admission on time. Late or missing signatures hold payment for months, and a missing notice forfeits days.
VisMed reviews coding and assessment with clinicians who know the grouping model, keeps a certification and notice calendar per patient, monitors visit thresholds during the period, and chases signatures from referring physicians on a schedule.
Where home health practices lose money
The patterns we find in the aging and denial history before anything else changes.
Primary diagnosis and comorbidity coding
Diagnoses that do not group, or comorbidities missed, place the period in a lower payment group or return the claim.Assessment functional scoring
The functional items drive the payment level. Inconsistent scoring under-pays or draws audit.Face-to-face and plan-of-care signatures
Payment waits for the physician’s signed plan of care and documented face-to-face encounter. Agencies without a chase process wait months.Notice of admission late
Filed late, days are forfeited from the start of care.Visit thresholds
Periods that fall below the visit threshold pay per visit, at a fraction of the period rate. Unmonitored, this happens by accident.

What we bill

Home health billing, asked and answered.
What a physician-owner in this specialty asks on the first call.
How do coding and assessment affect what we are paid?
Directly. The primary diagnosis, comorbidities and functional items determine the payment group for each period. VisMed’s clinical reviewers check them against the grouping rules before billing, so periods are paid at the level the care supports and the documentation defends.
How does VisMed get physicians to sign plans of care?
With a chase that starts the day the plan is sent, uses the physician’s preferred channel, and escalates on a schedule. VisMed tracks each referring practice, and the slow ones get a conversation.
Can VisMed handle Medicaid and managed-care home health?
Yes. Authorization, visit limits and per-visit or per-period rules differ per plan and VisMed holds them per payer, alongside the Medicare workflow.
What happens when a period falls below the visit threshold?
It pays per visit rather than at the period rate. VisMed monitors counts during the period and tells the clinical team so care planning is informed, not surprised.
How does VisMed handle a returned or rejected period claim?
Returned claims are worked the day they come back: the grouping, dates and notice data are corrected against the record and the claim resubmitted before the next period is due, because a stalled period holds the ones behind it.
