Sports medicine billing

Sports medicine bills procedures next to visits all day. The money is in the modifiers, the drugs and the guidance, and so are the denials. We bill it as the procedure practice it is.

01Our view

How we see sports medicine billing.

Reviewed by the VisMed primary & acute care billing team

A sports medicine visit often ends with a procedure: a joint or soft-tissue injection, aspiration, ultrasound guidance, a brace fitted, a fracture managed. Each is billable alongside the evaluation when the documentation supports it, and each has a rule that breaks it. The injection and the drug are separate lines. Guidance is billed only when images are retained. The brace is DME with a supplier number. Fracture care is a global package, or it is not, and the choice matters.

Viscosupplementation and biologics add authorization and buy-and-bill risk. Many plans require authorization per series and pay the drug at a rate that may not cover acquisition. A practice that does not check both before injecting can lose money on the procedure.

VisMed bills sports medicine as a procedure practice with a primary-care visit attached: modifier discipline, drug and guidance capture, DME enrollment, and authorization before every high-cost injectable.

02Where it leaks

Where sports medicine practices lose money

The patterns we find in the aging and denial history before anything else changes.

  1. Same-day visit and procedure

The evaluation that decides to inject is separately billable with modifier 25 when it is documented separately. Notes that describe only the procedure lose the visit.
  2. Drug and guidance unbilled

The injected drug, by units, and image guidance, with a retained image, are separate lines. Either missing is a straightforward loss on every injection.
  3. Viscosupplementation without authorization

Series injections are authorized per course on most plans. Without it, the drug, which the practice purchased, is not paid.
  4. Fracture care billed wrong

Global fracture care packages the follow-up visits; itemized care bills each visit. Choosing the wrong one, or billing follow-ups inside a global, is a denial or a recoupment.
  5. Braces and DME without enrollment

Orthoses need a DME supplier enrollment, HCPCS codes and proof of delivery. Without them, the practice gives the brace away.
03How we run it

What we do differently for sports medicine

Specialty-matched coders and a denials specialist, working inside your own system.

  1. We give you procedure-aware templates

Notes that separate the evaluation from the procedure, prompt for drug units, guidance and image retention, and record laterality and site.
  2. We authorize injectables before the appointment

Viscosupplementation, biologics and imaging are authorized against the plan’s criteria ahead of time, and we check the drug rate against your acquisition cost.
  3. We make the fracture-care decision at the first visit

Global or itemized is chosen per case with the payer’s rules, documented, and follow-ups are billed accordingly.
  4. We enroll you as a DME supplier and run the braces

Where needed we handle the supplier enrollment, then run braces through our DME workflow with delivery documentation.

05Questions

Sports medicine billing, asked and answered.

What a physician-owner in this specialty asks on the first call.

Can I bill a visit and an injection on the same day?

Yes, when the visit is a significant, separately documented evaluation and not just the decision to inject. VisMed builds templates so the two are distinct, and we check the note before the claim goes out.

How does VisMed handle viscosupplementation?

VisMed authorizes each series before the first injection, bills the drug by units with the NDC where required, and checks the payer’s allowed rate against your acquisition cost so you know whether buy-and-bill makes sense for that plan.

Should we bill global fracture care or individual visits?

It depends on the fracture, the expected follow-up and the payer. VisMed makes the call at the first visit with your physician, documents it, and bills follow-ups consistently with it.

We dispense braces. Can VisMed bill them?

Yes, provided the practice is enrolled as a DME supplier. VisMed can handle that enrollment and then run the braces with HCPCS codes, proof of delivery and any authorization the plan requires.

What does ultrasound guidance need to be paid?

A permanently recorded image and a note describing the guidance. Without the image the line is not defensible, so VisMed’s templates prompt for it and our coders will not bill guidance without it.
