Endocrinology billing

Endocrinology is chronic-disease management billed one visit at a time, with a growing layer of device and remote-monitoring revenue that most practices only partly capture. We run the device side as a program.

01Our view

How we see endocrinology billing.

Reviewed by the VisMed medical specialties billing team

The endocrinologist manages diabetes, thyroid disease and metabolic disorders over years, in visits that involve real decision-making and a lot of data review. That supports higher E/M levels than most practices bill. It also supports care-management and remote-monitoring codes, because continuous glucose monitoring and insulin pumps generate exactly the kind of data the codes were written for.

The device side is where the rules are newest and the errors most common. CGM has its own codes for placement, training, data download and interpretation, with frequency limits and payer coverage criteria. Pump training and management has another set. Practices that bill these ad hoc miss intervals, bill inside a frequency limit or lack the documentation the payer expects.

VisMed reviews the level on every chronic-management visit against the decision-making table, and runs the device and monitoring services as a program with intervals tracked per patient, so each interpretation goes out on the first eligible day and never inside a limit.

02Where it leaks

Where endocrinology practices lose money

The patterns we find in the aging and denial history before anything else changes.

  1. Complex visits leveled low

Insulin adjustment in a patient with multiple conditions is high-complexity decision-making. Notes that record the numbers without the reasoning level down.
  2. CGM interpretation unbilled or billed too often

Data interpretation is billable at defined intervals with a minimum data span. Without interval tracking, practices either miss it or exceed the limit and get denied.
  3. Device training and placement missed

Sensor placement, pump initiation and training are separately billable services that are commonly performed by staff and never charged.
  4. Infusions and injectables without authorization

Osteoporosis infusions, testosterone and specialty injectables are authorized per plan and per course. Missing authorization means the practice pays for the drug.
  5. Thyroid ultrasound and biopsy component errors

In-office ultrasound and fine-needle aspiration with guidance need images retained and components billed by ownership.
03How we run it

What we do differently for endocrinology

Specialty-matched coders and a denials specialist, working inside your own system.

  1. We review leveling on chronic management

Our coders read the assessment and plan against the decision-making table, with short feedback to the physician when the note undersells the visit.
  2. We run the device program on intervals

We track each CGM and pump patient’s last interpretation date, so the next is billed on the earliest allowed day and never inside a limit.
  3. We authorize infusions and injectables per course

Every course is authorized against the plan’s criteria before scheduling, and we compare the drug rate with your acquisition cost.
  4. We capture staff-performed services

Training, placement and download services performed by nurses and educators are prompted at charge entry and billed under the correct supervision rules.

05Questions

Endocrinology billing, asked and answered.

What a physician-owner in this specialty asks on the first call.

How often can CGM interpretation be billed?

At defined intervals per patient with a minimum span of data, and payers enforce it. VisMed tracks the last billed date for each device patient so the next interpretation goes out on the first eligible day and never early.

Are pump and sensor training billable when a nurse does them?

Yes, under the appropriate supervision and documentation rules, and they are among the most commonly missed services VisMed finds in endocrinology. Our charge-entry prompts pick them up.

Does VisMed handle authorizations for infusions?

Yes, per course and per plan, before the patient is scheduled. VisMed also checks the allowed drug rate against your acquisition cost so buy-and-bill decisions are made with the numbers in front of you.

Can VisMed run remote monitoring for us?

VisMed handles device enrollment, the monthly data thresholds, the clinical staff time and the claims, as a program run by our staff under your physicians’ supervision.

Will VisMed push our visit levels up?

VisMed bills what the note supports. In endocrinology the note usually undersells the visit, and the fix is a sentence of reasoning in the assessment, not a higher code by default.
