Neurology billing

Neurology visits are long, the tests are component-heavy and the drugs are expensive. Time and units decide most of the revenue, and both are easy to under-document. We capture both.

01Our view

How we see neurology billing.

Reviewed by the VisMed medical specialties billing team

Neurologists spend more time per patient than nearly any other outpatient specialty, which means time-based billing and prolonged services matter more here than almost anywhere. A ninety-minute new-patient visit that is billed as a standard level four gives away the difference.

Diagnostics add a second layer. EEG, EMG and nerve conduction studies are coded by duration, by number of studies and by technical and professional components, with bundling rules that trip up general coders. Infusions and botulinum toxin add a third: authorization per indication, units, wastage and NDC reporting.

VisMed’s neurology accounts get a time-and-level review on every visit, diagnostic coding by specialists who work from the report, and a drug program that authorizes, prices and reconciles every dose.

02Where it leaks

Where neurology practices lose money

The patterns we find in the aging and denial history before anything else changes.

  1. Long visits without time documented

Total time on the date of service supports the level and prolonged services. Without it, the visit levels on decision-making alone and prolonged service is lost.
  2. EMG and nerve conduction units

Nerve conduction codes step by the number of studies; EMG codes depend on the number of limbs and whether conduction studies were done the same day. Miscounting is a denial or a short payment.
  3. EEG duration and monitoring components

Routine, extended and long-term EEG have different codes by duration, with separate technical and professional components. Long-term monitoring adds daily and review-time rules.
  4. Botulinum toxin authorization and units

Approval is per indication, units are per vial with wastage rules, and many plans require the NDC. A treatment given outside those is a loss of the drug.
  5. Infusions for migraine and MS

Site-of-care rules, step therapy and per-course authorization apply, and the allowed rate has to cover acquisition.
03How we run it

What we do differently for neurology

Specialty-matched coders and a denials specialist, working inside your own system.

  1. We capture time on every visit

Templates record total time on the date of service, and our coder applies time or decision-making, whichever supports the visit, adding prolonged services where documented.
  2. We code diagnostics by rule

EMG, nerve conduction and EEG are coded from the report with the unit and component rules applied and matched to the payer’s edits.
  3. We run the drug program

Authorization per indication and course, reimbursement checked against cost, units and wastage reconciled to inventory.
  4. We cover the hospital and telehealth

Inpatient consultations, stroke and critical care time, and telehealth follow-up billed under the correct place-of-service rules.

05Questions

Neurology billing, asked and answered.

What a physician-owner in this specialty asks on the first call.

Should our physicians bill by time or by decision-making?

Whichever supports the visit, decided per visit from the note. In neurology, time often supports a higher level and unlocks prolonged services, but only if total time on the date is documented. VisMed’s templates make that a single line, and our coder makes the call.

Why are EMG claims being paid less than we bill?

Usually the nerve conduction study count or the EMG limb code was wrong for what was done, or the payer’s bundling edits applied. VisMed codes from the report with the unit rules and checks the edits before submission.

How does VisMed handle botulinum toxin?

VisMed authorizes per indication before treatment, bills units per vial with documented wastage and the wastage modifier where required, reports the NDC, and compares the allowed rate with acquisition so you know the margin.

Can VisMed bill long-term EEG monitoring?

VisMed applies the technical and professional components, the daily units and the review-time rules from the monitoring records, and builds the claims per the payer’s rules.

Does VisMed bill cognitive assessment and care planning?

Yes. The cognitive assessment code requires specific elements, a care plan and a defined amount of time, and it is commonly missed in memory clinics. VisMed gives physicians a template with the elements and bills it when the note supports it.
