How we see urgent care billing.
Reviewed by the VisMed primary & acute care billing team
Urgent care has no scheduled visits, so there is no time to verify eligibility in advance. The patient registers, is seen and leaves within the hour. Whatever the front desk captures in that window is what the claim is built on. A wrong subscriber ID, a missed secondary, an unrecorded copay: each becomes a denial or an unpaid balance weeks later, when the patient is a stranger.
On top of that, payers treat urgent care differently from each other. Some want a global S-code, some want E/M and procedures itemized, some pay a facility component and some do not. A practice that bills one way for everyone is wrong for half its payers.
VisMed runs urgent care as a real-time operation. We verify eligibility at registration, in seconds, so the copay is collected before the patient is roomed. We hold payer-specific claim construction per plan. And we split occupational medicine and workers’ compensation into their own workflows, because they follow their own rules.
Where urgent care practices lose money
The patterns we find in the aging and denial history before anything else changes.
Registration errors
A misspelled name, a transposed ID or an outdated plan captured at a busy front desk is a denial thirty days later. Urgent care has the highest rate of these in outpatient medicine because there is no pre-visit check.Wrong claim construction for the payer
S9083 global, S9088 add-on, or itemized E/M plus procedures: each payer has a preference and some reject the others. Billing one way for all is a systematic denial.Copays not collected at the desk
An uncollected copay at an urgent care is rarely collected afterwards. Statement costs can exceed the balance.Occupational medicine billed to health insurance
Employer physicals, drug screens and injury visits are invoiced to the employer or to workers’ compensation, with their own forms and timelines. Sent to the patient’s health plan, they are denied and time is lost.Procedures and tests unbilled
Splinting, laceration repair, X-ray, rapid tests and nebulizer treatments are routinely documented and not charged in a fast-moving clinic.
What we do differently for urgent care
Specialty-matched coders and a denials specialist, working inside your own system.
We verify eligibility in real time at registration
Our verification returns in seconds with the copay and deductible status, so your desk collects before the visit and the claim is built on verified data.We hold claim rules per payer
Global code, add-on or itemized, facility component or not: each plan you see has its construction rule in our system, applied at charge entry. We run occupational and workers’ comp as separate lanes
Employer accounts, protocols and invoicing are kept apart from health-plan billing, and a VisMed specialist handles state workers’ compensation forms and fee schedules. We reconcile charges the same day
Every registration must end as a claim with the tests and procedures documented. We close the list nightly, and unbilled procedures are the first thing our reconciliation catches.

Can VisMed verify eligibility fast enough for walk-ins?
VisMed’s real-time verification runs at registration and returns within seconds, with the copay and deductible status shown to the front desk before the patient is roomed.
Does VisMed know which payers want S9083 and which want itemized claims?
VisMed holds that rule per plan, and it drives claim construction automatically. When a payer changes its policy, we change the rule once and every subsequent claim follows.
We do a lot of employer and workers’ compensation work. Does VisMed include that?
Yes, as its own workflow. VisMed invoices employer accounts on their terms, files workers’ compensation claims on the state’s forms with the state’s fee schedule, and keeps neither mixed into health-plan billing.
How does VisMed handle the volume of small patient balances?
By collecting at the desk first, which is where the real fix is. What remains, VisMed posts accurately and statements cleanly, with card-on-file where your system supports it.
How fast do VisMed’s urgent care claims go out?
We reconcile charges the same day and scrub and submit claims daily. Urgent care cash flow depends on it, so nothing waits for a weekly batch.
